Feature Audit: Access Icons

An icon is displayed based on fetch state: unrequested, requested, fetched, failed, unpermitted, or unfetchable. It also wires the click handler so the icon can refresh or refetch the document.

Currently this only shows for http resources, not https.

It shows for NamedNodes only. So this is not something that appears in the storage pane.
