* docs(triage): the record closes with the releases that carry it

* docs: V1 reads fails=0, and every row of the ledger closes

* docs: the front matter uses the vocabulary the structure check defines

`status: closed` is not one of `active | landed | superseded | abandoned`, so
both records declared no valid status and the index was stale. Both are
`landed`, and `.agents/docs/README.md` is regenerated.
