1.2.8: reliable Claude Desktop restart, package layout

- install.ps1 finds Claude Desktop by its package family (Start menu, then
  the package list) and works out how to start it before stopping it; when
  it cannot, Claude is left running. Per-user installs start through their
  launcher. The restart question shows the version again.
- pack.ps1 resolves -OutDir to a full path, so release packages no longer
  nest their files under xpp-lens/xpp-lens/dist/xpp-lens
