Adds v0.1.19 to v0.1.21 to the upgrade page's "Release Notes That Need Action".

• Upgrade the gateways before the CLI. A v0.1.19+ CLI can't run commands on an older gateway: the attached run's placeholder start needs no MFA only on v0.1.19+, so an older gateway refuses it with "Multi-factor authentication is required for this action" before any prompt. A pre-v0.1.19 CLI can't run commands on a v0.1.19+ gateway either. During a rollout, run commands on each rack with the CLI that matches its gateway.
• Keep the CLI and the gateway on the same release for --script. v0.1.21 changed the runner, which the gateway recognizes a script command by.
• require_command_justification and the Slack patterns for commands.
