require_command_justification was only a per-app setting, so a rack that wanted a justification for every command had to set it on every app, and an app added later had none. It's now a gateway-wide setting too. The per-app one stays.
Behaviour
A command (run, exec) needs a justification when the gateway-wide setting is on or its app's own setting is. An app can't opt out of the gateway-wide one.
Commands under a deploy approval are still justified by the approval's message.
A stored value that isn't true or false fails closed, as the app setting already did.
Changes
Settings: a global require_command_justification key in its own group, saved at PUT/DELETE /api/v1/settings/require-command-justification. Changing it needs MFA on the request (MFAAlways), like the app setting. RGW_SETTING_REQUIRE_COMMAND_JUSTIFICATION sets it from the environment. GetRequireCommandJustification(app) reads both settings.
CLI:run and exec read /api/v1/settings as well as the app's settings. Either way, a command without a justification is refused before any MFA prompt or process start. Otherwise the gateway's refusal of an exec only shows up as websocket: bad handshake.
Web: a Command Justifications card on the Settings page. The Destructive Actions card became a shared GlobalToggleCard that both use. When the gateway-wide setting is on, the app's Command Justifications card says that the app requires one whatever its own setting says.
scripts/deploy.sh: its migrate run now passes a --justification, so deploys keep working with the setting on.
Docs: the CLI justifications section, settings page, environment variables, audit logging, permissions (MFA levels), the API reference, and v0.1.24 upgrade notes. The notes say to turn the setting on once users' CLIs are on v0.1.24, because an older CLI reads only the app setting.
Version bump to v0.1.24.
Testing
Settings service: either setting is enough, an app can't opt out, the environment variable works, and an invalid stored value fails closed.
Proxy: TestRequiredCommandJustification runs for both the app setting and the gateway-wide one.
CLI: exec and run are refused before anything is sent, for both sources.
Web: the Settings page saves and clears the gateway-wide setting at its endpoint, and the app card shows its note only when the gateway-wide setting is on.
Mutation checks, each caught by a test: ignoring the gateway-wide value in the service, in the CLI, and on the fail-closed path, and hiding the app card's note.
task go:lint, web lint and typecheck are clean. The Go unit tests for settings, rbac, proxy, handlers, routes and cli pass, and so do all 192 web unit tests.
Summary by CodeRabbit
New Features
Administrators can require command justifications across all apps with a gateway-wide setting. When enabled, it applies even if an app’s own setting is off, and changing it requires MFA.
App settings indicate when the gateway-wide requirement applies. The CLI and gateway enforce required justifications for commands.
Documentation
Updated setup, security, and upgrade guidance, including compatibility notes for older CLI versions.
require_command_justification is now a global setting as well as a per-app one.
With the global setting on, every command (run, exec) in every app needs a
justification, whatever the app's own setting says: either one is enough, and an
app can't opt out of the global one.
- settings: the global key and its own group, saved at
/api/v1/settings/require-command-justification (MFA on the request, like the
app setting). RGW_SETTING_REQUIRE_COMMAND_JUSTIFICATION sets it from the
environment. GetRequireCommandJustification reads both, and fails closed.
- CLI: run and exec read the global settings as well as the app's, so a command
without a justification is refused before anything starts either way (the
gateway's refusal of an exec only shows as "websocket: bad handshake").
- Web: a Command Justifications card on the Settings page (the Destructive
Actions card became a shared GlobalToggleCard), and the app's card says when
the global setting requires one.
- scripts/deploy.sh gives its migration run a justification.
- Docs: commands, settings, environment variables, audit logging, permissions,
API reference and upgrade notes for v0.1.24.

Walkthrough
The gateway adds a global require_command_justification setting. The CLI and gateway check it alongside app-level settings. Admins can manage the setting through the web interface, and app settings indicate when the global requirement applies.

Global setting and API internal/gateway/settings/*, internal/gateway/handlers/settings.go, internal/gateway/routes/route_registration.go, internal/gateway/rbac/*, docs/src/content/docs/development/api-reference.mdx
The global setting defaults to false. Its update and delete routes use the command-justification setting group, which requires MFA.
CLI and gateway enforcement internal/cli/convox_process_approval.go, internal/cli/convox_env_protected_test.go, internal/cli/convox_process_command_test.go, internal/gateway/settings/service_commands.go, internal/gateway/proxy/command_justification*, scripts/deploy.sh
The CLI checks the global setting before the app setting. The gateway requires a justification when either setting applies. Tests cover both scopes, and the migration command supplies a justification.
Web settings controls web/src/pages/app-settings-page.tsx, web/src/pages/app-settings/command-justification-card*, web/src/pages/settings-page*, web/src/pages/settings/global-toggle-card.tsx
The Settings page adds a global justification toggle. App settings display a notice when the global requirement is enabled.
Release and usage documentation convox.yml, web/package.json, docs/src/content/docs/{concepts,configuration,operations,security,user-guide}/...
The gateway image and web package versions advance to 0.1.24. Documentation covers global and app-level requirements, command behavior, MFA, and upgrade guidance.

Explanation
The title "feat: gateway-wide require_command_justification" names the main change. The PR adds a gateway-wide command-justification setting that applies to run and exec in every app, alongside the existing per-app setting. The changeset includes the settings service, the RBAC and MFA rules, the HTTP routes, the CLI checks, the web Settings page, and the docs, all of which match the title's subject. The title follows the conventional commit style and stays under 50 characters. It does not describe the version bump or the deploy script change, which is expected.

Build API paths with apiPath, and pass the DELETE query through params.
The mandatory rule applies to both paths. The proposed PUT change is correct. The proposed DELETE change must pass key through the request configuration instead of embedding the query in apiPath.

Update the settings path construction in the component to use the `apiPath`
helper with the normalized setting slug, and pass the DELETE request’s `key`
through the request configuration’s `params` instead of embedding it in the URL.
Apply the API path convention to both PUT and DELETE requests.
