Rewrite Rules: Flush rewrite rules on the next request after plugin changes

Summary

• defer rewrite rule flushing until the next request after plugin activation and deactivation
• mark active plugin updates for the same deferred flush path during upgrader_process_complete
• add unit coverage for local and multisite plugin change markers
