fix: match Claude connector setup instructions to the current dialog - #373

The custom connector dialog in Claude Desktop and claude.ai asks for a name and URL first and reserves the authorization header while OAuth sign-in is selected. The instructions now say to choose "No sign-in" and add the authorization header, and show the server URL and the header value as separate values to copy.

Problem
The Claude setup instructions added in #371 don't match the "Add custom connector" dialog in Claude Desktop and claude.ai:
/mcpanswers 401 without a token, so the dialog preselects OAuth sign-in ("Sign in now"). While OAuth sign-in is selected, theauthorizationrequest header is reserved and can't be added.Changes
