Expand review and reliability workflows

Summary

• add opt-in AI code review with validated, stale-guarded findings and branch-start review baselines
• support Azure DevOps review-thread replies and resolution plus provider-aware squash/rebase branch cleanup
• add paired Pierre syntax palettes, updater-safe Windows taskbar icons, and stale Vite cache recovery
• update roadmap, durable learnings, README, website demo, and user guide for the shipped behavior
