feat(dump): vertical metadata CSV, system columns in data dictionary - #35

The metadata CSV is now a Field / Value pair per row instead of one wide header row and one value row, so it reads top to bottom in a spreadsheet. The data dictionary (CSV and JSON) now describes the system columns the data download carries: `_id` first and `_updated_at` last (when INCLUDE_UPDATED_AT is on). A schema with no fields is left as is. `datastore_info` is unchanged.
