Fix the Storage Plugin Helper Instance Types table - #691

The table has had a header with no rows since the page was added in 71e49c8, with its three rows spilled below it as a single escaped line. Restore them as real rows, each with the condition the helper picks that instance type for. Also rename the storage_create argument default_conf, which does not exist, to default_type.
