fix: stream partial chunks, then one aggregated response - #1721

- Streaming now ends with one aggregated, non-partial response; read either the partial chunks or the final response, not both. - The aggregated response carries the finish reason, and an error code and message when the stream did not end normally. PiperOrigin-RevId: 1008786956
