fix(server): MCP create replays respect the target's current mode - #26354
A create_threads or schedule_task retry with the same clientRequestId acts on the thread or task that the first call created. Only the new request's modes were checked against the caller's ceiling. The user can raise a thread's or task's mode after creation, so a restricted caller (for example a Supervised thread) could replay the call and act above its own modes:
create_threads: a replay that adds a first prompt dispatched it to the thread created earlier. If the user had raised that thread to Full access, the restricted caller started a turn under the elevated policy.
schedule_task: a replay upserted over the existing task, so a restricted caller could overwrite or disable a task the user had raised above its ceiling.
Fix
create_threads: the first-prompt message.dispatch now runs under DispatchModeLimit with the caller's modes, as ThreadLaunchService and task cancel already do. The orchestrator rechecks the thread's current modes under its command lock and refuses with runtime_mode_escalation_denied / interaction_mode_escalation_denied. A prompt the orchestrator already accepted still replays from its receipt unchanged, so ordinary retries return the original result.
schedule_task: before saving over a replayed task, it runs the check update_scheduled_task uses on that task's current run modes (factored out of loadScheduledTask). The MCP service now passes the task id it derives from the request key, so the check and the upsert target the same row.
The schedule_task check, like update_scheduled_task's, happens before the save rather than under a lock. The window is a user raising the task during the call itself.
Tests: the existing create_threads replay case now creates a thread, raises it, and expects the prompted replay to be refused with no message or run. A new case in the scheduled-task mode suite does the same for schedule_task. Both fail on main.

A create_threads or schedule_task retry with the same clientRequestId acts
on the thread or task the first call created, but only the new request's
modes were checked against the caller's ceiling. After the user raised that
target, a restricted caller could prompt the thread or overwrite the task
above its own modes.
create_threads now dispatches the first prompt under DispatchModeLimit, so
the orchestrator rechecks the thread's current modes under its lock. A
prompt already accepted still replays unchanged. schedule_task checks a
replayed task's current run modes with the same check update_scheduled_task
uses before it saves over it.

Scheduled-task loading and replay now enforce caller mode limits. Prompt dispatch for created threads also runs within the parent thread’s runtime and interaction-mode limits.

Scheduled-task mode validation and replay apps/server/src/mcp/OrchestratorMcpService.ts, apps/server/src/mcp/OrchestratorMcpService.test.ts
Scheduled-task loading checks each task run mode against caller limits. Requests with a clientRequestId derive stable task and command IDs, check any existing task against those limits, and then upsert. Tests verify that replaying a request after raising the saved task’s runtime mode is denied without a second upsert.
Prompt dispatch uses the parent thread’s runtime and interaction-mode limits. Mode-limit errors map to escalation-denied failures. Tests verify that a replay is denied after raising the child thread’s runtime mode, without creating messages or runs.
pingdotgg/t3code#22770 — The issue covers runtime-mode escalation for scheduled-task operations. This PR adds a caller-limit check when replaying a schedule_task request against an existing task.

The current test raises only runtimeMode, so it cannot detect removal of the interaction-mode check. Add a caller narrowed to plan, save the task once, then raise only its saved interaction mode. The replay then passes the runtime check, returns interaction_mode_escalation_denied, and leaves the upsert count unchanged.

Exercise interaction-mode escalation in this replay test.
The current case only raises runtimeMode. The parent remains at interactionMode: "default", and the child inherits that mode, so no child can exceed the interaction limit. Add a separate plan-parent case, raise the child to default, and replay its prompt with the same clientRequestId. Restore the parent to default before the existing settings and provider assertions.
