fix: keyless shim for decide_triage (doctor honesty) - #15

Doctor claimed decide works keyless via shim, but ShimDecider raised without an LLM. Add a deterministic uncalibrated heuristic path so new --from-example decide_triage + run reaches the human gate (exit 2).
CI lint failed on the keyless shim PR; .env.example still said type: decide was unshipped after Phases 2–3 landed on main.
