It covers the gating layers, the human-only actions, the commands that currently pass the PreToolUse chain unguarded, and the places where claude-config's own docs no longer match the hooks. Nobody could say what the personify pipeline and the agent gating actually do today without reading a dozen scripts across three repos, and the one mapping attempt died before it reported. This adds a dated record of both, checked against source on 2026-09-23.
The unguarded paths were found by piping command strings through the live hook chain, not by running them. The stale-docs table lists only mismatches read on both sides; fixing those docs belongs in claude-config and is not done here.
