Tracking issue: #1401

Part of the file-write-safety series (#1375) — B3a: per-step change cards + the changeCardDetail setting (extension host). Stacked on B2.

Scope note: the planned B3a (cards + rollback + setting) measured 1484 changed lines — over the series' 1000-line hard cap — so, per the plan's budget rule, it splits into sequential sub-PRs: this PR ships the cards + setting; the rollback service ships as a stacked follow-up (#1410), and the webview chat UI (card rendering, rollback buttons) ships as B3b. The changeCardDetail settings control ships in THIS PR as part of the persisted-setting round trip (the AGENTS.md checklist requires the SettingsView control, cachedState binding, save payload, and locales in the same PR as the setting); B3b owns the chat-side UI only.

Budget note

Raw diff as of 67d852598: 1579 insertions / 67 deletions (44 files) — over the series' 1000-line hard cap, like #1412. Breakdown: ~972 lines of per-tool parity wiring and its focused specs (six write tools each need their own checkpointSave call site + spec: write_to_file / edit_file / apply_patch / apply_diff from the base scope, plus edit + search_replace added in round 12 after the trial-review audit found those two default-exposed tools wrote with no checkpoint / journal entry / change card), ~535 lines of change-card emission + checkpoint specs, ~194 lines of shared types / setting round trip, 72 lines of i18n parity (CI check-translations mandated, 18 locales). The overage is concentrated in the mandated parity scope (every successful file write must produce a checkpoint + journal entry + card per epic #1375 AC) and the mandated i18n lines; the PR remains atomic and mergeable (merge-tree clean vs upstream/main).

What

  • Per-step change cards (payload + emission). When a tool step finishes its writes, checkpointSave emits one change_card message carrying a typed ChangeCardData payload (checkpointIds, files: [{ path, additions, deletions, diff? }], totalFiles, detail: "full" | "summary") — schema + types added to the shared package. It reuses the unified diff and diff stats the tools already compute for approval: WriteToFile / EditFile / ApplyPatch now thread their approval diff, stats, and the real auto-approval decision into the checkpoint write info — nothing is recomputed. Auto-approved steps get the card too (informational, emitted after the fact, non-interactive). Card emission failures are logged and never disable checkpoints.
  • changeCardDetail setting ("full" | "summary", default "summary"). Full round trip per the persisted-setting checklist: global-settings.ts (DEFAULT_CHANGE_CARD_DETAIL + zod enum) → ExtensionState → ClineProvider.getState() + getStateToPostToWebview() (shared default) → webview initial state. summary = one-line card + per-file list (the payload still carries the diff; whether to render it lazily is B3b's UI decision); full = per-file diff inline. Auto-approval steps always get the compact card regardless of the setting.
  • i18n label + description for the setting in all 18 locale files (check-translations parity).

Tests

  • Card payload for a multi-file step: counts, fields, diff present for full / omitted for summary; auto-approved step still emits the card; a failing card path never disables checkpoints.
  • Tool plumbing: approval diff/stats + the auto-approval decision flow through on all six write tools (write_to_file / edit_file / apply_patch / apply_diff / edit / search_replace — the last two added in round 12 after the trial-review audit found they wrote with no checkpoint, journal entry, or change card).
  • Setting round trip: getStateToPostToWebview returns the saved "full"/"summary" and the default when unset.
  • Local gates: src tools suite green (30 spec files, 608 passed incl. the new editSearchReplaceTool.changeCard.spec.ts and the updated editTool/searchReplaceTool specs), eslint 0, tsc 0, 100% of the executable changed lines covered (verified against the local lcov report), i18n parity green (CI check-translations).

Hardening (post-review)

  • The per-write checkpoint is now awaited before execute() settles in the apply_patch and edit_file tools, so a settled execute implies a recorded checkpoint (and its change card). Regression tests hold the checkpoint promise and prove execute waits.

kind: branch - feat/change-cards-b3a, refreshed onto current main by merge 39c1eff64 (upstream/main 9c069e94ef); formatting-only follow-up c72617cce.

Budget (own delta at this head)

Own delta vs the B2 head (86f41828b): 48 files, +1751 / -131. The GitHub stacked view is larger until B1/B2 merge; merge order: B1 (PR 1404), B2 (PR 1406), then this PR.

node scripts/find-missing-translations.js

Expected: all green; src/eslint-suppressions.json unchanged.

Verification at this head (ce72f5cc8)

  • CI: 18/18 checks green at head (incl. compile with the new format:check step).
  • Local: ESLint --max-warnings=0 clean on all 25 touched src files; tsc --noEmit clean; src vitest 571 passed / 5 skipped / 0 failed (13 files); webview-ui vitest 63 passed (3 files); translations check clean; model-check 9/10 locally (10th = known local Node 25 + tsx env defect only; green in CI).
  • Prettier: clean on every file in the unit diff. src/eslint-suppressions.json: unchanged.

Updated this round (refresh onto main 9c069e94ef)

  • 39c1eff64 - merge upstream/main. Two test-file conflicts resolved keeping all three test groups: this PR's changeCardDetail round-trip tests, B1's perWriteCheckpoints tests, and main's new blanket-auto-deny tests; ExtensionStateContext.spec.tsx takes main's DEFAULT_WRITE_DELAY_MS placeholder shape and keeps the perWriteCheckpoints / changeCardDetail keys.
  • c72617cce - formatting-only: the new format:check gate flagged three files in this diff (the two changeJournal files inherited from B2 plus this PR's applyPatch execute spec); reformatted with prettier --write, byte-parity with the B2 fix verified via git hash-object.

Update (CodeRabbit-sync from trial #1413): head 9b0787aaa — createInitialExtensionState exported + focused test asserting the pre-hydration changeCardDetail default (trial addendum 4fc14c4). Review context: trial PR #1413.

Update (User-feedback-sync from trial #1413): head 67d852598 (341a9c2 + CodeRabbit round: the pre-hydration defaults spec now asserts both the changeCardDetail and perWriteCheckpoints initializer defaults; + trial-review finding: the edit and search_replace tools (exact string replacement, exposed by default) wrote with no per-write checkpoint, no journal entry, and no change card - the B3a wiring now covers them with the same single checkpointSave call site as write_to_file / edit_file / apply_patch / apply_diff, plus a focused 8-test spec editSearchReplaceTool.changeCard.spec.ts) (94fea2f + CodeRabbit round: the pre-hydration defaults spec now asserts both the changeCardDetail and perWriteCheckpoints initializer defaults) - User-feedback addendum (trial #1413 review): apply_diff writes now record a per-write checkpoint and emit the per-step change card (parity with write_to_file / edit_file / apply_patch) - previously apply_diff wrote with no checkpoint, so its edits had no restore button in chat. New focused spec applyDiffTool.changeCard.spec.ts (4 tests). (trial: #1413)

Review-gate re-trigger (2026-08-30): empty commit 0d31ea0 (no code change) re-runs CI and CodeRabbit current-head review under the org new PR review gate; the code head remains 8c5c53b.

Visual regression note (2026-08-30): The advisory extension-host-visual check (chat-dark sidebar snapshot) fails on this branch with a deterministic 973-pixel delta localized to the completion-result area. Root cause: the B1 task-start baseline checkpoint (per-write checkpoints default on, allowEmpty) adds a suppressed checkpoint_saved row to the message history, which getCompletionCheckpoint() picks up, so SeeNewChangesButtons render below the completion row; the suite baseline (recorded pre-FWS in #1426) has no such buttons. Every other pixel is identical, and the delta is pixel-identical across all branches containing B1 (1404/1406/1410/1411/1412/1413), while non-B1 branches are green. This is a new-expected-behavior vs stale-baseline mismatch rather than a rendering regression. Flagged for maintainers: either update the electron-chat-dark-sidebar.png baseline on the B1 branch or adjust the scene/component behavior. Advisory check only — it does not block the required gates.

  • 9633e181a - review follow-up: Turkish (tr/settings.json) changeCardDetail disabled-state sentence fixed per the review thread (possessive/comitative link added: ...satır sayılarıyla birlikte dosya listesini gösterir.); JSON validated, translations parity check clean, Prettier clean; identical fix ported to the sibling units carrying the key (PR 1410 5f881bffa, PR 1412 ae70bedf2).

Pre-merge check dispositions (head 9633e181a)

The walkthrough's failed rows are design-contract rows on stacked content; dispositions with pointers, argued once here:

  • Persistence Integrity (error) - journal appendFile "can lose state": by design, not a defect. The change journal's crash-safety model is append-then-repair, not temp+rename: appendChange is a single fs.appendFile syscall (minimal torn-write window, src/core/checkpoints/changeJournal.ts:40-47), and loadChanges discards a torn final line and parses the rest without throwing (changeJournal.ts:70-90). The repair path is directly tested (torn-tail truncation discarded, remaining entries parsed, absent/empty journal returns [] in src/core/checkpoints/__tests__/changeJournal.spec.ts). A torn tail loses at most the one in-flight entry - the checkpoint itself is already committed before the append - which is the documented trade-off of the append-only design owned by PR 1406 (B2). Rewriting the journal via temp+rename per entry would add a full-file rewrite per write without removing the loss window.
  • Regression Evidence (warning) - Playwright visual test for the checkpoints settings: the repo's test-placement guidance (AGENTS.md) puts settings controls, forms, and binding at the webview-ui vitest layer, which this PR extends (CheckpointSettings.spec.tsx: default-on binding, saved-true/saved-false bindings, toggle caching; SettingsView.spec.tsx for the cachedState round trip), and the electron chat-dark sidebar reference is regenerated in this branch. No Playwright component-test suite exists in-repo for settings controls today; adding one here would be a new harness outside this unit's scope.
  • Out of Scope Changes (error) - webview settings control in this changeset: the stated-scope sentence was stale and is corrected above: the settings control is owned by this unit (persisted-setting checklist requires the full round trip in the same PR as the setting), and the B3b description has been aligned to claim only the chat card UI + rollback buttons. No code moves.
  • Lifecycle Resource Cleanup (warning) - checkpoint overlap: the write path has no fire-and-forget: every write tool awaits checkpointSave inline after the write (EditFileTool.ts:488, same call-site shape in write/apply-patch/edit/search-replace), so within a task, checkpoint work cannot overlap itself. The task-start baseline is guarded by taskStartBaselineDone and awaited before the first request (regression-tested in Task.spec.ts across two loop invocations). Suppressed saves do not touch UI state. This is the same conclusion reached on the B1/B2 rounds; no code change requested by the model-check suite (check-task-cleanup-protocol, check-task-lifecycle green).
  • c3bef96f7 - review follow-up (ported from PR 1404): task-start baseline is now skipped on history resumes (initiateTaskLoop gates on _isHistoryTask), matching the exactly-once-per-task design; regression test added in Task.spec.ts. Task.spec 177/177 green, tsc clean, Prettier clean.
  • f73f38c5d - review follow-up (ported from PR 1410): baseline skipped on already-aborted tasks (regression test added; Task.spec 178/178), and WriteToFileTool/EditFileTool now read perWriteCheckpoints through the shared DEFAULT_PER_WRITE_CHECKPOINTS constant. tsc clean, eslint clean, Prettier clean.
  • 525c023e2 - review follow-up (ported from PR 1410): the baseline block rechecks this.abort after the getState() lookup, so cancellation during the deferred state lookup skips the baseline save; regression test added (Task.spec 179/179, negative control verified). tsc clean, eslint clean, Prettier clean.
  • 578d8fb24 - review follow-up (queued-message handoff, raised on PR 1404): the per-file handlers processed queued messages before execute awaited the whole-patch checkpoint, so a queued turn (and its own checkpointed writes) could start mid-commit. The handoff is now deferred to after the checkpoint attempt - one handoff per successful patch, independent of the checkpoint setting or outcome - matching the deferral the journal-carrying unit already shipped. Tests: held-promise deferral (negative control verified), handoff with checkpoints disabled, no handoff on failure. Tool specs green, tsc/eslint/Prettier clean.
  • e0f15d20d - port: aborted tasks no longer dequeue queued messages (handoff sites guarded at the Task layer after the awaited-checkpoint change); regression test added. Task spec 180 passed.
  • 2ef457376 - Stryker-directive sweep: removed the seven carried // Stryker disable next-line comments in Task.ts's streaming/telemetry region (comment-only). Equivalences: event.name always a string on those paths; streamingKey never undefined there; getStreamingKey fallback unreachable defensive code. eslint/tsc/Prettier clean.
  • 1ee917c56 - refresh onto upstream/main (58341d8 lint cleanup) and fix the unused-variable fallout the merge exposed: dropped the unused ChangeJournalEntry import in the checkpoint index, renamed unused test stub arg (_uri) and two unused positional destructures (_partial, _sayOptions) in the checkpoint-save spec. Full-src eslint 0, tsc 0, checkpoint specs 16 passed, Prettier clean.
  • The delete-patch fixture now uses canonical unindented patch-marker lines, per the review thread. The delete flow was already parsed (parseOneHunk trims each hunk's first line), but the fixture is now canonical; verified the delete path is genuinely exercised (breaking the tool's delete branch reddens the delete-path tests). eslint/tsc/Prettier clean.
  • Refreshed onto upstream/main (tool-call state scoping): ToolCallbacks gained a required toolCallId; the execute and change-card spec callback fixtures pass it (test-tool-call-id, matching main's own spec convention). No production-code change. Full-src eslint 0, tsc 0, execute/checkpoint/Task/change-card specs green (248 passed).
  • The change-card Zod contracts are now tested directly in the types package: valid full and summary cards parse; invalid detail values, incomplete file entries (missing path/stats, non-numeric stats), and payloads missing checkpointIds/files/totalFiles/detail are rejected (negative control: widening detail reddens exactly the invalid-detail test). packages/types tsc 0, message.test 12 passed, Prettier clean.
  • Preemptive port of the sibling unit's no-op guard: change-card emission is skipped when the writes array is empty (a no-op patch still checkpoints; unrelated workspace changes can produce the commit, and a zero-file card would misrender); the no-op tool test asserts the empty payload and an emission-layer test covers the guard (negative control: removing the guard reddens exactly that test). eslint/tsc/Prettier clean, specs 38 passed.

Security Boundaries walkthrough row - fixed

Port of the sibling-unit fix: journalPath validates the task ID as a single safe path segment before any filesystem operation (separators, traversal, absolute paths, empty and dot-only IDs rejected), so a crafted ID cannot move changes.jsonl outside <globalStorageDir>/tasks. Regression test covers journalPath, appendChange and loadChanges rejection plus containment of safe IDs. Negative control: removing the validation reddens exactly that test, restored byte-exact.

Persistence Integrity walkthrough row - fixed

Port of the sibling-unit fix: torn-tail repair + per-append fsync in appendChange with regression tests (negative control: disabling the repair reddens exactly the torn-tail test). The whole-file atomic-rename redesign is not adopted: the journal is advisory metadata, the checkpoint commit is the durable artifact, loadChanges tolerates torn lines by design, and append failures are logged rather than silently dropped.

  • Review follow-up on the journal hardening: the tail-inspection catch now suppresses only ENOENT (fresh journal) and propagates other stat/open/read errors, and the append loops on FileHandle.write until the whole record is written before syncing (a short write can no longer persist a torn entry under a reported success). Two regression tests; negative controls each redden exactly their test, byte-exact restore. eslint/tsc/Prettier clean, checkpoints suites green.
  • Review follow-up on the journal: loadChanges validates each parsed line at runtime (non-objects, missing string path/checkpointId, and unknown operations are skipped, matching the sibling units), and on POSIX the first append syncs the parent directory so the newly created journal's directory entry is durable (Windows skipped). Two regression tests; negative controls each redden exactly their test, byte-exact restore. eslint/tsc/Prettier clean, checkpoints suites green.
  • Persistence Integrity row - implemented as demanded: journal updates are now one serialized (per-task queue), atomic replacement per checkpoint - read current journal, repair a torn tail, write the complete next content to a temp file, fsync it, atomically rename onto changes.jsonl, and fsync the parent directory (POSIX; Windows cannot open directories). checkpointSave persists the whole step's writes in that single update and propagates journal failures instead of reporting a fully durable checkpoint with lost entries; the change card is emitted only after the journal update is durable. Save failures keep the historical handler (logged, checkpoints disabled, resolves undefined). Failure-injection tests: post-commit journal failure (no card, checkpoints stay enabled), mid-update rename failure (all-or-nothing journal, temp file cleaned up), concurrent-update serialization (no lost updates), restart recovery from a stale temp file plus torn tail, and empty updates. Negative controls: dropping the propagation, the queue, the temp cleanup, or the directory sync each reddens exactly one test (12 NCs across the series), restored byte-exact. eslint/tsc/Prettier clean; checkpoints suites and the apply-patch execution suite green.

📝 Summary

Summary by CodeRabbit

  • New Features
    • Added automatic checkpoints after successful file writes, enabled by default, plus a baseline checkpoint when a task starts.
    • Added per-step change cards showing changed files and added/deleted line counts, with an option to include full diffs. Auto-approved changes use compact summaries.
    • Added settings to control per-write checkpoints and change-card detail.
    • Change cards can summarize multi-file edits.
  • Localization
    • Added translated settings text across supported languages.

The change adds per-write checkpoint settings, a per-task change journal, and per-step change_card messages. Task startup can save a baseline checkpoint. File-writing tools submit write metadata, and the webview exposes the new settings.

Merge Risk: 🟡 Moderate · up to ce72f

The change cards and checkpoint journal mostly look sound. Before merging, check that a partially failed patch still checkpoints the files it already wrote, and that the webview exposes and saves the change-card detail setting. On Windows, a file scanner holding the journal open could occasionally fail a write step.

Full details: Regression Evidence

Explanation

The regression remains unresolved. CheckpointSettings.tsx adds two durable visible VSCodeCheckbox controls for perWriteCheckpoints and changeCardDetail (lines 46-73), and SettingsView.tsx renders them. CheckpointSettings.spec.tsx provides Vitest interaction tests with HTML test doubles, but the reviewed range adds no Playwright component test or checkpoint-settings __screenshots__ baseline. The only changed image is the unrelated Electron chat sidebar snapshot.

Resolution

Add a Playwright component visual test and fixture for CheckpointSettings. Cover representative unset/default and saved states with supported VS Code themes. Commit the generated checkpoint-settings __screenshots__ baselines. Keep the existing Vitest interaction tests.

Full details: Persistence Integrity

Explanation

The journal writer is now serialized and atomic, but the changed tool paths still discard journal failures. checkpointSave commits first, then awaits appendChanges and throws on failure (src/core/checkpoints/index.ts:260-295). WriteToFileTool and ApplyDiffTool explicitly catch and ignore that rejection (src/core/tools/WriteToFileTool.ts:213-221, src/core/tools/ApplyDiffTool.ts:254-260). The file write and checkpoint can therefore succeed while changes.jsonl and the change card are missing, with no checkpoint ID or affected write set reported. The other new tool callers route the exception to generic error handling without exposing partial-persistence metadata. A blocked or failed journal directory is a concrete trigger.

Resolution

Propagate a typed partial-persistence error or result from checkpointSave containing the committed checkpoint ID and complete write set when appendChanges fails. Do not use .catch(() => {}) at tool call sites. Ensure all six write tools await the result and report the partial persistence state before continuing, or retry journal persistence before reporting success. Add tool-level tests that force journal failure and verify that the write is not reported as fully successful and that the checkpoint ID and affected paths are surfaced.

Full details: Lifecycle Resource Cleanup

Explanation

The prior lifecycle failure remains unresolved. Each changed write path can enter checkpointSave (for example WriteToFileTool.ts:213-221 and ApplyPatchTool.ts:221-240). checkpointSave then awaits ShadowCheckpointService.saveCheckpoint and performs journal and change-card work without checking task.abort, disposal, or task instance (src/core/checkpoints/index.ts:260-309). ShadowCheckpointService.saveCheckpoint stages and commits without a task-ID operation lock (src/services/checkpoints/ShadowCheckpointService.ts:331-377), while RepoPerTaskCheckpointService maps replacement instances to the same tasks/<taskId>/checkpoints directory (src/services/checkpoints/RepoPerTaskCheckpointService.ts:6-13). Task.abortTaskOnce starts dispose without awaiting it (src/core/task/Task.ts:3515-3521), and ClineProvider.cancelTask creates the replacement after awaiting only abortTask (src/core/webview/ClineProvider.ts:3484-3507, 3594-3595). A cancellation during a pending per-write save therefore allows the old task to continue staging, committing, and updating the shared journal while the replacement task starts work. The journal queue serializes journal replacements only; it does not serialize checkpoint commits or card emission. The changed tests cover checkpoint ordering and queued-message retention, but not cancellation followed by replacement.

Resolution

Serialize the complete checkpointSave lifecycle by task ID, including shadow-Git save, journal append, and change-card emission. Track active per-task work and have Task.abortTask() and dispose() await or cancel it before replacement. Add abort/disposal and task-instance guards before per-write checkpoint work and before journal/card work. Add a replacement test with a pending checkpoint save and assert that the old task cannot commit, append, or emit after cancellation.

119-170: 🗄️ Data Integrity & Integration | 🟠 Major | ⚡ Quick win

Flush earlier writes after an access denial.

If an earlier hunk writes a file and a later validateAccess() call returns false, Line 135 returns before the post-loop checkpoint code. The written file then has no checkpoint, journal entry, or change card.

Set patchSucceeded to false and break the loop instead of returning. This preserves the recorded successful subset.

Proposed fix
if (!accessAllowed) {
   await task.say("rooignore_error", relPath)
   pushToolResult(formatResponse.rooIgnoreError(relPath))
- return
+ patchSucceeded = false
+ break
}

Add a regression with one successful hunk followed by an access-denied hunk. As per coding guidelines, “For regressions, add the test at the lowest layer that would have failed.”

createInitialExtensionState is exported (shared with the sibling pre-hydration default tests) and a focused test asserts it initializes changeCardDetail to summary: a regression that dropped the key from the initializer would otherwise stay hidden because the merge tests supply the key manually (CodeRabbit finding on trial Zoo-Code-Org#1413).

Invoke changeCardSchema/changeCardFileSchema/changeCardDetailSchema
with a valid full and summary card, and reject invalid detail
values, incomplete file entries, and incomplete payloads.
Port of the sibling-unit fix: a no-op patch still reaches
checkpointSave with an empty writes array, and unrelated workspace
changes can produce the commit; emitting a card for it would show
a change card with zero files. Skip emission when the writes array
is empty (the checkpoint itself still runs), assert the empty
payload in the no-op tool test, and cover the guard at the
emission layer.

Pre-merge checks failed. Please resolve the failing checks before merging.

Port of the sibling-unit fix: a crash can leave the journal
without a trailing newline; the next append previously spliced
the new entry into the fragment and lost both at load time.
appendChange now terminates a torn tail before appending and
fsyncs each append so a durable checkpoint implies a durable
journal entry. Task IDs are validated as a single path segment
before any filesystem operation so a crafted ID cannot move the
journal outside <globalStorageDir>/tasks.
… writes

appendChange previously swallowed every tail-inspection error,
so an I/O failure could let the append proceed without the
required separator and silently lose the entry to a merged
line; only ENOENT (fresh journal) is suppressed now. The
append also ignored FileHandle.write's bytesWritten, so a
partial write became a durable torn record reported as
success; the loop now writes the whole record before syncing.
Regression tests cover both paths.

Comment thread src/core/checkpoints/changeJournal.ts Outdated

loadChanges now skips parsed lines that are not entry-shaped
(non-object, missing string path/checkpointId, unknown
operation) instead of returning them as entries; consumers
read path/checkpointId unguarded and a null element made
rollback throw. On POSIX, the first append also syncs the
parent directory so the newly created changes.jsonl entry is
durable after a crash (Windows cannot open directories and is
skipped). Regression tests cover both.
Implement the pre-merge Persistence Integrity demand: appendChanges
replaces the per-entry append with one serialized (per-task) atomic
update - read current journal, repair a torn tail, write the complete
next content to a temp file, fsync it, atomically rename onto
changes.jsonl, and fsync the parent directory (POSIX; Windows cannot
open directories). A crash leaves either the old or the new complete
journal, never a half-applied multi-file update. checkpointSave now
persists the whole step's writes in that single update and propagates
journal failures instead of reporting a fully durable checkpoint with
lost entries; the change card is emitted only after the journal is
durable. Save failures keep the historical handler (logged, checkpoints
disabled, resolves undefined). Regression tests cover post-commit
failure, mid-update rename failure (all-or-nothing, temp cleanup),
concurrent-update serialization, restart recovery from a stale temp
file and torn tail, and empty updates.

🩺 Stability & Availability | 🟡 Minor | ⚡ Quick win

Handle a Windows rename onto an existing journal.

After the first append, changes.jsonl already exists. On Windows, fs.rename onto an existing destination can fail with EPERM or EBUSY. This happens when another process holds the file open, for example an antivirus or indexer scan. When the rename fails, the catch block removes the temp file and rethrows the error. checkpointSave then rethrows, so the write tool's step fails and no change card is emitted. Before this change, the flow used append-only writes and did not hit this failure. Retry EPERM/EBUSY/EACCES a few times with a short backoff before you propagate the error. Keep the destination in place. Do not unlink the destination first, because that breaks atomicity.

Based on learnings: "On Windows, Node.js fs.rename() can throw EPERM/EEXIST when the destination path already exists."
