Related work and prerequisites

Part of #1948. Extracts the file-read safety boundary from #1950 without closing the feature issue or introducing the dependent batch tool.

Document-buffer decoding prerequisite #2002 is already merged into main. Symlinked-workspace approval matching has now been extracted into the independently mergeable prerequisite #2041.

Exact stacked history

Based on main at 1d37994, with exactly two commits:

  1. 8cf4cd8 — the exact single commit from fix(auto-approval): match symlinked workspace roots #2041, with its SHA retained unchanged.
  2. 3f8be95 — one commit containing all remaining verified-reader, cancellation, legacy, image-helper, and regression-test changes.

The first commit owns twelve auto-approval files (+655/-52 lines). The second changes only the remaining nine reader-safety files (+2154/-188 lines). The nine safety-owned source/test files are byte-for-byte identical to this PR's previously published head 342f3db. The approval prerequisite additionally incorporates the UNC review fix from #2041; the identical amended prerequisite SHA is retained in both PRs.

Merge #2041 first, then update this PR onto main so the approval prerequisite drops out of its diff. While the prerequisite is unmerged, this branch deliberately carries its exact commit plus one safety commit.

Reader safety and cancellation scope

  • Capture the canonical target and bigint device/inode identity before approval. Validate both the requested path and resolved target, and classify real workspace roots so an internal alias to an external file receives the actual external approval policy.
  • Open approved targets with no-follow protection where supported, compare descriptor identity, consume bytes through that same descriptor, and close it on every exit path. Reject canonical-file, final-symlink, and parent-directory replacement without disclosing replacement bytes.
  • Consume descriptor-bound ordinary text, supported image bytes, and PDF/DOCX/IPYNB/XLSX bytes. The positioned binary probe does not consume the subsequent payload. Decoders use the already-merged buffer boundary from refactor(extraction): decode documents from required buffers #2002; no extraction-module changes are included here.
  • Observe existing task cancellation flags through a task-bound cancellation object and checked asynchronous operations. A typed cancellation error retains its original identity and stack when propagated.
  • Honor cancellation in BOTH native and legacy readers, including pre-cancelled calls, approval, metadata, probes, image processing, provider lookup, text reads, tracking, cleanup, result publication, and later files in a legacy batch.
  • Discard late content and late technical failures without incorrectly setting read-failure or user-rejection flags. Preserve already-received approval feedback, including image-only feedback. Already-started non-cancellable work cannot be rolled back; its late result is discarded and acquired descriptors are still closed.

Compatibility and explicit behavior change

Keep the public reader interface/exported singleton, ordinary string formatting, legacy multi-file/range/image behavior, and uncancelled legacy text fallback when binary probing fails.

Legacy cancellation is intentionally corrected: it no longer keeps reading after the task is cancelled. The previous description's claim that legacy cancellation policy was unchanged no longer applies.

The identity check pins the approved file, not a content snapshot: in-place writes to the same inode are not target replacement.

No batch tool, raw-document API, numeric/range overhaul, formatter/strategy architecture overhaul, prompts, settings, persisted lifecycle transitions, dependency changes, changesets, or changelog edits are introduced by the safety commit.

Verification

Validated the rebuilt stack on Node 22.23.1 and pnpm 10.8.1 with a frozen install of the dependencies from current main:

  • 706 focused tests passed in 27 suites, including reader, cancellation, real-filesystem safety, extraction compatibility, image mentions, and approval policy.
  • The public safety integration suite contains 142 scenarios, including descriptor cleanup, native/legacy cancellation, late errors/results, received feedback, symlink policies, and approved-byte handling.
  • Full monorepo test command passed: 13/13 Turbo tasks, including 10,795 backend tests passed and 39 skipped.
  • Monorepo lint and type checks passed: 11/11 tasks each. Scoped ESLint pruning did not change suppression counts; whitespace checks passed.
  • The lifecycle model-check umbrella passed. This remains compatibility evidence for its documented bounded protocols, not a claim that the models prove file-reader behavior.
  • Exact 1+2 commit counts, the identical prerequisite SHA, disjoint 12+9 file ownership, and byte-for-byte safety-source preservation were checked programmatically.
  • Normal commit and push hooks remained enabled. The old published head is retained in a local backup, and the history update used an explicit lease against that head.

Remote CI and review must rerun against this new history; local results do not claim remote approval.

📝 Summary

Summary by CodeRabbit

  • Bug Fixes
    • File reads now verify that the approved file is still the same file before accessing its contents, reducing the risk of reading a changed target.
    • Cancelled or abandoned reads no longer return partial file contents or images, and stop before starting further processing.
    • Text, image, and document extraction use data from the verified open file, including spreadsheet, PDF, DOCX, and notebook files.
    • Missing files and other read failures continue to surface as errors rather than returning incomplete results.

File reads now resolve canonical targets and use verified open handles for content access. Cancellation checks cover approval, file operations, and result construction. Text, image, and document extraction can process data from open handles or supplied buffers.

Changes: File reading

Layer / File(s) Summary
Resolve targets and handle approval
src/core/tools/file-reading/ReadFileTarget.ts, src/core/tools/file-reading/readFileCancellation.ts, src/core/tools/ReadFileTool.ts, src/core/tools/file-reading/__tests__/readFileSafety.integration.spec.ts
ReadFileTarget captures canonical paths and file identity. ReadFileTool checks access and approval against resolved paths and records approval feedback.
Read and process file content
src/core/tools/ReadFileTool.ts, src/core/tools/helpers/imageHelpers.ts, src/integrations/misc/extract-text.ts, src/integrations/misc/extract-text-from-xlsx.ts, src/core/tools/__tests__/readFileTool.spec.ts, src/integrations/misc/__tests__/*, src/core/tools/file-reading/__tests__/readFileSafety.integration.spec.ts
Modern and legacy reads use open handles for text, image, and document content. Extraction functions accept buffers. Image helpers accept open handles for reads and file stats.
Stop cancelled reads and build results
src/core/tools/ReadFileTool.ts, src/core/tools/file-reading/__tests__/readFileSafety.integration.spec.ts
Cancellation stops later file operations and processing. Cancelled results discard file content and retain available approval feedback. Tests cover cancellation across asynchronous operations.

Full details: Regression Evidence

Explanation

The new external-approval classification has an uncovered unset-workspace branch. ReadFileTarget.isOutsideWorkspace() now treats vscode.workspace.workspaceFolders ?? [] as outside the workspace, but every safety integration case sets a non-empty workspace folder list (lines 165-167; the only variation uses a symlinked non-empty root at lines 394-396). No focused read-file test verifies behavior when workspaceFolders is undefined or empty, although this changed path controls whether approval is bypassed or requested.

Resolution

Add a focused integration test with vscode.workspace.workspaceFolders set to undefined or []. Read an approved file and assert that the approval payload has isOutsideWorkspace: true, manual approval is required, and content I/O starts only after approval. Restore the workspace-folder mock after the test.

Thanks for contributing. This comment tracks the review sequence and the next action.

Current step: Fix the failing required CI checks; awaiting-maintainer requires CI and automated review completion.

Review-state labels are managed by this workflow; do not edit them manually. community-approved is managed the same way — do not add or remove it manually. It signals a fresh community code approval for the current head as an advisory priority only; maintainer review is still required.

🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win

Assert the extractor bytes and path.

The PDF and DOCX tests only assert that extractTextFromBuffer was called. Their fixed mock responses allow incorrect buffer bytes or fullPath metadata to pass. The test instruction requires value assertions when those values are verifiable. Assert the expected Buffer and canonical path for each call.

Extract document decoding from PR Zoo-Code-Org#1999 as an independent prerequisite on main. Keep explicit path-only adapters and mandatory-buffer PDF/DOCX/IPYNB/XLSX decoding, preserve formatted output, and separate workbook formatting. Load XLSX buffers directly with precise typing and exact subarray boundaries. Include real PDF/DOCX fixtures and full per-file coverage, align the backend coverage provider with Vitest, and remove redundant conditions and legacy lint suppressions.

Build on the original document-extraction prerequisite from PR Zoo-Code-Org#2002. Preserve the remaining verified-descriptor, canonical-target approval, cancellation, legacy compatibility, image-helper and symlink-root allowlist changes from PR Zoo-Code-Org#1999, including the reviewed diagnostic/privacy fixes and regression coverage. …e-Org#2002)

* refactor(extraction): decode documents from required buffers

Extract document decoding from PR Zoo-Code-Org#1999 as an independent prerequisite on main. Keep explicit path-only adapters and mandatory-buffer PDF/DOCX/IPYNB/XLSX decoding, preserve formatted output, and separate workbook formatting. Load XLSX buffers directly with precise typing and exact subarray boundaries. Include real PDF/DOCX fixtures and full per-file coverage, align the backend coverage provider with Vitest, and remove redundant conditions and legacy lint suppressions.

* chore(extraction): keep coverage dependencies unchanged

* test(extraction): make document fixtures reproducible

Canonicalize verified task-local workspace root spellings through platform-specific strategies, preserve ordered exclusions and lexical-parent rules, and log resolution failures without widening read or write permissions.
Keep the symlinked-workspace approval prerequisite unchanged. Pin approved targets to verified descriptors, consume descriptor-bound text/document/image bytes, close resources on all paths, and honor cancellation in both native and legacy readers while preserving received approval feedback.
